Issues like "chronic food insecurity, early childhood malnutrition and nutrition insecurity" continue to plague North Korea, which has been ranked 98 out of 118 countries in the 2016 Global Hunger Index, according to the report put together by five United Nations agencies, seven worldwide nongovernmental organizations and the Swiss Agency for Development and Cooperation.

The US military has observed activity in North Korea that suggests Pyongyang may be gearing up for another nuclear test, a US official said Thursday.

The report said that the sanctions have also contributed to the drying up of humanitarian assistance and relief and appealed for donations for millions of North Korean women and children.

South Korea's state-run think tank on diplomacy on Saturday called for a temporary hike in the nation's defense budget to secure sufficient deterrence against North Korea's nuclear weapons program.

He added that South Korean and USA military have determined that whenever Kim Jong-un makes a decision, another nuclear test can be conducted at either at the No. 2 or No. 3 tunnels at the Punggye-ri nuclear test site.

"The Human Rights Council spoke with one voice today by condemning North Korea's horrific rights abuses and supporting efforts to bring leading officials in Pyongyang to account", said John Fisher, Geneva director.

The sanctions, it said, also have a psychological impact on the donors, making them reluctant to provide funds for projects in the country. The experts will be able to develop plans for the eventual prosecution of North Korean leaders and officials responsible for human rights crimes.

"This is reflected in the radical decline in donor funding since 2012", it said.

On Monday, Reuters quoted a USA official as saying that Washington was considering more sweeping sanctions as part of a broad review of measures against North Korea.

North Korea has been raging against the world with missile launches and nuke blasts as tensions rise in the Korean Peninsula.

Washington has recently angered Pyongyang by starting the installation of an advanced missile system at an air base in South Korea.
